TIPS FOR FILLING OUT THE FORM
- Enter the complete address of the shipper and the recipient. Address labels or address stamps may not be used to enter this information. Please note that the destination country must be stated in either German or English.
The telephone number and/or e-mail address of the recipient is required for item notification. By providing it, you help prevent potential delivery delays. If the information is included, please ensure that the recipient has consented to the sharing of their data.
- For parcels only: If you would like your shipment delivered faster (fee-based PREMIUM/PRIORITY service), please check the relevant box.
- Please indicate if your shipment should be returned to you if it cannot be delivered. A service fee is charged for the return of undeliverable parcels. If you do not want such parcels/small packages returned, they will be auctioned or destroyed in the destination country.
- Please indicate the category of item by putting an X in the appropriate box. If your item does not match any of the categories listed, please select “Other”.
When sending items in the category “Sale of goods”, please note that the customs tariff number and the country of origin of your goods must be indicated in the next step.
Failure to indicate the category of item can result in the return of your shipment. Returns are subject to a service charge.
- Please enter a detailed description of each item contained in your shipment. Identical items can be listed in a single row with a single product description and their total number entered in the “Quantity” column. If your shipment contains different kinds of items, please select a new row for each product group.
When providing a detailed description of shipment contents, please note the following: Collective terms such as “clothes” are not allowed.
Example of a correct description of contents: “Cotton t-shirts”.
Failure to provide a description of the contents can result in the return of your shipment. Returns are subject to a service charge.
- The customs tariff number and the country of origin must only be provided on the form if you are shipping items in the category “Sale of goods” – i.e., goods that are the subject of a business transaction (for example, an internet auction). In this case, the customs tariff number – also called the HS Code, a six, eight or ten-digit code assigned using the Harmonized System – and the product’s country of manufacture must be indicated. You must also include a pro forma invoice with the shipment.

The value of items in a row may not be indicated as “0”.

- Enter any special provisions to which your shipment may be subject. These are needed in case the contents fall under separate import and export regulations and/or require documents, permits, certificates or approvals. Mark the appropriate fields and provide the relevant descriptions and/or identification numbers of the documents. Submit any required documents together with your shipment at a retail outlet. Our staff will send the documents along with your parcel.
Instructions, tips and information
Fill in all of the mandatory information completely and correctly as instructed above. Missing, incorrect, unclear or unreadable information can lead to a return of the goods, transit time delays or similar inconveniences. Returns are subject to a service charge.
The information you provide on the dispatch note should be legible and written in print (no cursive) using a black or blue ballpoint pen.
Languages
Before you start, check which languages are permitted on the dispatch note in the country of destination. Information on that topic can be found at dhl.de/laenderinfo. Failure to fill out a customs declaration in an appropriate language can lead to a return of your shipment. Returns are subject to a service charge.
Small parcel shipping (goods value up to ca. EUR350)
Fill in sections 1 to 5 completely and post your shipment with the completed dispatch note at the counter of your local retail outlet. The duplicate page does not have to be filled out.
Parcel shipping
Fill in sections 1 to 5 completely. Information on the original will also be printed on the carbon copy. The copy does not have to be filled out again (provided it is readable). If necessary, please complete the information in section 6 and post your shipment with the carbon copy (customs declaration CN23) at the counter of your local retail outlet.
Import restrictions
Be aware of any potential important restrictions in the destination country that may apply to your shipment and make sure that you are not shipping dangerous goods (e.g., spray cans, perfume, lighters, nail polish or batteries).
